Formula Update

The formula shortage is impacting residents in our communities. In response to this, PDHD has developed a survey for organizations to submit if they have formula available. The public is able to see these responses in real time. Please use the most recent response from an organization as they may fill out the survey more than once due to supply changes.

Many parents are being forced to change their infant’s formula due to the shortage. Changing an infant’s formula may cause them temporary digestive upset that should subside in two to three days.  Please call your doctor if your infant develops projectile vomiting, or a rash.

Please DO NOT make homemade formula and DO NOT give dairy milk to children under 12mos old.

You can buy ANY formula with SNAP, if available.


WIC Participants please call PDHD clinics if you need your formula changed. We will change it while you are standing in the store. Hours are Monday through Thursday 7:30-4:30 and Fridays 7:30-11a.

Purchase District Health Department Vaccine Update 11/09/2021 (Pfizer)

Purchase District Health Department will begin to administer COVID-19 Pfizer vaccines at all PDHD clinics on 11/15/2021. Pfizer vaccines are by appointment only. We are accepting calls to schedule an appointment beginning on 11/10/2021. This includes initial vaccinations, boosters, adolescent, and pediatric Pfizer COVID-19 Vaccines. Pfizer has been approved for use in 5-11 year olds. A parent or legal guardian must be present for anyone under 18 years old. Please be patient as call volumes will be high.

PDHD clinics are appointment only for all COVID-19 vaccines (Pfizer, Moderna, and J&J):

Purchase District Health Department COVID-19 Vaccination Update 10/28/2021

Purchase District Health Department Vaccine Update

Beginning November 1st all Purchase District Health Department Health Centers will start administering J&J and Moderna COVID-19 vaccine boosters. You must be 18 years or older. Vaccinations are no cost to you with insurance or through a state program for the uninsured.

J&J Booster Eligibility:

  • Two months after initial immunization
  • Ages 18 and up
  • It is recommended that you receive the same vaccine as you did initially, but mix and match is allowed.

Moderna Booster Eligibility:

  • Booster should be at least six months after full vaccination
  • Ages 65+
  • Ages 18-64 with underlying health conditions or at high risk for occupational or institutional exposure.
  • It is recommended that you receive the same vaccine as you did initially, but mix and match is allowed.

Moderna Third Vaccine for Immunocompromised Eligibility:

  • Third shot should be at least 28 days after full vaccination.
  • Receiving active cancer treatment for tumors or cancers of the blood;
  • Received an organ transplant and am taking medicine to suppress my immune system;
  • Received a stem cell transplant within the last 2 years or am taking medicine to suppress the immune system;
  • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, WiskottAldrich syndrome);
  • Advanced or untreated HIV infection;
  • Active treatment with high-dose corticosteroids or other drugs that suppress my immune response.

  • Carlisle: Appointment only. Please call (270) 628-5431.
  • Fulton: Appointment preferred. Fulton City Location Only: Mon-Th 7:30a-4:30p. Please call: (270) 472-1982. Staff will come to your vehicle to administer if you are handicap. Please inform staff when calling.
  • Hickman: Appointment only. Call: (270) 653-6110. On November 9th ONLY, vaccine will be available through drive up on location, no appointment needed.
  • McCracken: Schedule will vary. Please visit for most up to date schedule. Expect wait times. When vaccines are administered at Drive Up: No appointment needed. Please drive up to the tent in the parking lot behind the health department at 916 Kentucky Ave, Paducah KY 42003. When vaccines are administered in clinic, appointment preferred. FREE COVID-19 Testing

FREE COVID-19 Testing is still available at the Purchase District Health Department. Schedule will vary. Please visit for most up to date schedule. Testing is drive-up, no appointment needed. Ages 12 and up ONLY. This service is free to you. Go directly to the Drive-Up tent.  If you are tested you will receive a call with your results. Please do not call or come into the clinic for test results. We will call you once we receive your test results.

PDHD Drive Up location is 916 Kentucky Ave, Paducah KY 42003 in the back parking lot at the tent when scheduled at this location.  Testing is only available by drive up on scheduled days. Do not enter PDHD clinics for test or test results.

Information Regarding Restaurant Grade Posting

On August 1, 2013 the Purchase District Health began implementing a new policy that requires restaurants in Ballard, Carlise, Fulton Hickman and McCracken counties to post a letter grade that corresponds to their health department inspection score.
